This is one of the best opt-in form I have ever seen.
Also see the call to action to the facebook like...

This is on a self hosted video, and was right at the end.
I am not sure this can be done on youtube hosted video, as YT will overlay this scene with the related videos default scene I guess.
But I thought I will share. I know, no one will take action except maybe one or two..
 


Ya, social on a squeeze page i think is worth testing. Why not? (Change out facebook to twitter etc)
Maybe also test just asking for email not name to see if you can get more opt-ins.

Try different words in the Submit box. (and colors)
 
YT has an API for custom players, not exactly sure though if it would allow you to do something like that.. speaking from a design perspective, the image in OP could be improved I'd bet for a better ctr.
 
Seeing the play button in there makes me think the like button and opt-in form were 'inside' the video. Is that right blogspotter?

Not even saying it would do better. The latter one just adds the presupposition that they "liked" the video , since they made it to the end they either loved it or hated it. If they hated it well, it's still a good ad. The last thing you ever want is a neutral position from your visitors . Leaned that from the late Gary Halbert

Actually, if you add this code to the video, it will not show related videos:

&rel=0

So, say your video URL was
Code:
http://www.youtube.com/watch?v=gBiOp4AnT5A
then you can just change it to
Code:
http://www.youtube.com/watch?v=gBiOp4AnT5A&rel=0

I'm pretty sure that's what you were talking about.

Thanks for that/.. it is actually very useful.

Also, I was thinking, it would actually be easy with youtube as well.. Just make that screen run for 30 seconds or so before you cut off the video.. :-)

Now off to find someone to have some create that flash scene..
 
LOL. I never took the time to review Easy Video Player's advanced features. Only used it for basic player skins... no controls... etc.

Looks like it's pretty easy to add the opt-in and do it in a way where it's timed with EVP. It's these features below:
