Starting your own Adsense type network

The thought has crossed mine and I'm sure many other webmasters' minds.
What features would you look for as a 1) publisher and 2) advertiser?
What improvements would you ask for?
Are there any existing scripts out there that can be improved on?

As a publisher, I would want the rules as much as in line with Google except for their content restrictions. I want the ads fully customizable (color, size, font) and a version like Google's adlinks except the pages the PPC ads are on can be customized to look like my site. The ability to turn of contextual targeting would be nice too so I can stick it on pages with Adsense.

As an advertiser I would want to able to block my ads from showing up on specific sites, and have fraudulant click data made available without running the traffic through a third party.
