Facebook is automatically recording the # tag of the image, so let's say that they really don't like a specific image; they record the # tag, and anytime that same image is submitted, it automatically becomes dis-approved. To change the # tag of an image, just add a border, change the color; even increasing the size of your image by 1 pixel up or down will change the code each time.
Along with this, they track similar # tag on different accounts. If you use the same image on 2 accounts, then they know that these two accounts are being run by the same individual. So if one account is subject to be disabled, both of them will.
Remember to keep an separate folder for each account, and slightly change the # tag for each image for each account.
