LinkShare - Tax Forms?

Hey all,

I'm new to this whole affiliate game - just started my website about 2 and a half weeks ago. I've got a partnership set up with Amazon right now, but I'm looking to expand to other clothing retailers but I'm not sure how other than by directly e-mailing them.

I read about a site called LinkShare and thought it'd be a good idea to join it, but it asks me for Tax Form information... so I'm a bit stumped and not sure what to do.

Any help is appreciated!
