I suck at design. That being said I probably design 50% of my websites myself, and use templates for the other 50%.

When I do create something from scratch, I usually try to find 3-4 websites with a similar style to what I'm shooting for, just as a creative reference. Fireworks is a lifesaver for me, I do better work when I can mock up a design in fireworks and slice it up.
 
Wordpress or really cruddy Photoshop---->Imageready---->Dreamweaver template since I'm not too fly with that stuff. I have a few "goto" wordpress templates I tend to like... but I'm looking for some more simple, SEO friendly, clean and sharp templates with a regular header at the top and everything pretty status quo.

Or, for a big project, get some personal recommendations/references, look at some portfolios and evaulate. You gotta cough it up for quality.
 
I make my own and sometimes edit (optimize) existing WP-themes for my MFA-sites. I'm pretty good at photoshop/css/etc. But I hate it that I can't code any decent php app or so.

With building and spreading your own WP-themes you can get a nice amount of backlinks.
 
design everything myself (when it's a long term site)...I'm more a webdesigner trying to get into AM than else
lately I'm often going with wordpress and free templates because I just want to have it online fast and don't care of it being unique etc

I always used to design my own, but am now leaning towards outsourcing. I have a few guys who have great design skills I can call on. If I see a template for sale or in a contest on shitepoint I save the designers details ...

Any recommendations for good template sites I can yank shit down from? I really wish wordpress could act like a CMS in the sense that it could seamlessly operate in 'website mode' as opposed to 'blog mode' (and by that I mean have options to support pages and subsections and stuff and not all be in terms of entries)
 
I have a design guy and pay him by hour. It's not bad if you can't do everything by yourself, but be honest and don't try if you suck at it. It's well worth the money and much more effective than spending endless hours to learn it yourself.
 
For plain HTML or ASP/ASP.NET sites I do my own designs from my CSS library. Most of these are based on common examples like 'jello mold'. I've got rather large clip art and photo art collections that I can drop into Photoshop to create logos and stuff. I don't do a lot of PHP so I usually use a template for WordPress or Joomla and apply mods to the style sheet as needed.

Time is an investing tool that Ive got a constant supply of. I dont mind spending a few weeks to get a halfway decent design together so i can learn CSS as well as have something ready for the future. All the while, it saves me money which I dont have to earn back with every site.

I have a couple of templates I have made myself, quite ugly and fairly basic, just one tiny banner graphic in the whole site and one for background.
The navigation allows me to add pages very quickly, also via simple php includes I can turn a site around within 5 minutes, regardless of its size.
The basic template is nothing more than a collection of php includes.
So long as the site then runs on autopilot to bring a couple of steady daily dollars I am more than happy.
