Test/quiz script?

Does anyone know where I can find a good test/quiz script (PHP or JavaScript doesn't matter)? I'm looking for something like this:

You can do this with simple HTML. For Quiz's that I use on my sites, I just scrape a similar quiz I find on the interwebs and then modify it for my needs(questions, graphics, links).

Yeah been trying to do the same thing. I was thinking of doing with Formstack but it only does calculations for multiple choice and true/false. See if it fits your needs.
