Pre-poppable in-house insurance affiliate programs?

I'm looking for an in-house insurance affiliate program (not part of a CPA network nor CJ/Linkshare/etc.) that lets you pre-pop at least the zip code, and who allows you to place a tracking pixel. Host and post is fine, too.

I'm already aware of SureHits, but they pay per click, which isn't what I'm looking for. Prospectzone too but their payouts seem a little low. And most of the rest I've found aren't an in-house program.

Any suggestions besides makemoniesonline?

Leadpile allows you to create your own form/path, but generally the payouts are based on an auction system - and that can sometimes mean a low payout per lead.

Yeah, I'm actually signed up with Leadpile already. I find any lead exchange has payouts and sell rate that are too inconsistent and much prefer fixed price.

hey satoshi, how many leads/week do you generate? For what types of insurance? If it's a good amount I can refer you to some of our host and post customers and see if they're interested. PM me if you want. They have good fixed price CPL.

We work with in-house programs, so they're not on any network...
