Website is approx 6 months old. Worked hard on our content and On-Page SEO using Yoast Pro with keyword and key-phrases, passed all Yoast's tests. I know not to expect instant ranking but nothing has happened on ANY of the KWs. Today I check the site using SEO-Quake KW Density and see that the ONLY KWs listed are internal Wordpress related words, file and plugin names, etc. That list goes on and on, hundreds of them.

What causes this? What can I do?
