Need Help Creating a Simple but Nice Looking Form

I want to create a relatively simple "Quote" form for an html lander my friend is using, but I don't want it to look old and tired like most forms. I'm just using a cgi mail script for this so it needs to be able to work with that.

This is his site Get Cash for Your Used Car in Orange County & the Inland Empire. If the page looks familiar it's the lander that Bloghue gave out a while ago.

The form he's got on there right now is just a free one from ContactMe.com but they don't give you access to the raw html, so the input button can't be changed etc.

Ideally, something that looks exactly like this would be perfect, but I just haven't done enough forms in the past to really create something decent.

If anyone has any good suggestions for how I could duplicate a form like this or if someone want's to build it for me, I'd be happy to pay if you want to PM me with a price. Thanks.
 


I re-worked the form at the link you provided, which you can download here: efeezy form.zip

The form looks identical, and the CTA button copy can be updated by changing the HTML source (edit the "value" of the submit button). The CTA button is now css'd with a background image, so it will auto-scale itself with copy you want on the button.
