Need a payment processing form for WP

Simple, no frills - I just need a payment form that will go in Wordpress. The user needs to be able to enter their own payment amount as it's for an offline company to allow their customers to pay their bill online.

It will process though Authorize.net and needs a captcha field.

Who can do it, how fast and how much?
