This is the original theme, which I now own:

I want to use this theme for a Loans website, and instead of the video you see, I want a blended "OPT-IN" box.

I will create all the content & I've already uploaded the theme to my domain: And it looks like this so far....

but its bare so far... I need someone who is capable of making it like the original optimize theme you see above, except with my content & an OPT-IN box where you see that video.

Well I'd start off by uploading the theme you want it to look like.

You've uploaded "coffee break" but you want it to look like "optimize".
 
You mean the JS slider? I don't see a video. Are you going to use Aweber?

I can customize Wordpress themes with ease. Hit me up.
