Looking for a WP plugin that allows to amend and download a doc/pdf file

Hi all,
I'm looking for a plugin that will allow a user to insert 1-2 variables (date, name) and in return he will receive a file ( either downloadable either to an email he enters) which is a pre-made invitation form (pdf/doc) that used to have two unkonwn variables, but now it includes the info that the user entered.

Hope I managed to be clear and will appreciate the help!
(Basically I want the user to be able to customize a greeting/birthday card, by entering several variables, and being able to download the result).

Your requirements make sense, but I don't think you're going to find a plugin that does exactly that out of the box. I think it would have to be custom done. Shouldn't be that expensive though.

Gravity forms has an add on or extension that can do this.
