Local Affiliate Contract -- Need a sample

After reading Shoemoney's blog and some of the posts here I went out and scored a Local affiliate deal. It's simple. I deliver the leads, he pays me for it. I have a verbal agreement from the guy and I know he's good for it, but I need to cover my ass and put something in writing. I looked everywhere for a sample contract like that, but can't seem to find anything that fits.

ANy of you guys got who have done a local deal have a sample contract? Or perhaps can point me into the direction where I can get one? I don't mind paying for it.

Present yourself as a customer to a competitor. Ask to see a copy of the contract so you can read it over.
 
A contract won't amount to a hill of beans if the local business refuses to pay you for "unqualified" leads. Have you taken a look at his sales processes and validated them to make sure he isn't going to blame you for his crappy salespeople not being able to close them?
 
It depends if you are doing basic "CPA" based leads, or just a flat monthly fee, which ultimately comes down to what kind of businesses you are targetting. I do alot of local marketing, PM me if you need some generic forms we use.

I'm a customer of of this compnay, so I know they can sell. Also, one of the reasons I do need a contract in place, so that both parties are clear on what a "qualified Lead" is. Will need to spell it out so that there are no surprises at the end of the day....
 
I'm still looking for the sample of that contact, so if anybody had used one, please let me know
