How can I add a optin page and custom contact info page to WP site

I got a WP blog site up for someone for auto repairs and wanted to add a optin page to collect emails and contact info for clients or possible clients. Whats the best way to go bye doing this are there any plugins already to do so? Is there a way to add a page so customers enter there car info and make a appointment for there car. I have seen this in some other sites with a page form to enter the info I guess the info is sent to the shop owner with the car info and sets a appointment. I also want to catch emails for anyone that clicks and adds there email I think its called a opt-in page and add it to the blog.

It be great if there are plugings ready made for this if so could you recommend of any thanks.

If you wish to collect emails of everyone who comments you can try - WP OptIn - This will allow you to add a "Subscribe to newsletter" tickbox in your WP

If you need more complex forms, you'll need some intermediate knowledge on how forms work and then it should be pretty easy to have it set-up in a custom page of your choice.
