Building a lead generation site

Hi All,

I need a lead generating site.
On the front end lead forms (a few of them)
on the backend, each form should send the data to a few sources (for example - emails) plus keep track of everything (while giving access to affiliates to see how many leads they brought to this site).

Does anyone know a good system for that?
