- A copy of a government-issued photograph identification (i.e. passport,
driver's license) that provides date of birth.

- A copy of a utility bill (cannot be an online statement), cell phone
bill, bank statement, credit card statement, 401K or Brokerage statements
verying the address. We are unable to complete verification using a PO Box
address. The document must have been issued within the last year and
contain the name of the PayPal account holder and the residential address
listed on the PayPal account.

- A copy of your social security card, Paystub, IRS statement, Military ID,
W2 or any other official document with the name and SSN displayed.

You can upload scanned documentation or print a fax cover sheet in the
Resolution Center. The Resolution Center can be accessed by logging into
your PayPal account.

Nope. Went through this a few months ago. Gave them a NV DL, my soc, and a CT utility bill, and all was well. If your documents don't agree, just explain why and you should be fine.
 
Everything's cool, had to send this stuff too (minus the SS because I'm outside of US) and everything was ok. But if you are using fake name:

Somewhat related question: If I'm underage now (while still having paypal) and then submit my ID when I'm 18, will paypal ban me?

I'm more worried about all my fake paypal accounts lol

edit: BTW thank you for the responses guys, when I was researching this on google i got some horrible answers (i thought matt cutts was suppose to improve this shit). i swear recently forums and youtube are my new search engines

Nope, you're fine.

I got one of those this past winter when I moved and changed my address. One call to their customer service and everything was sorted.

Nothing to worry about, just give them a call.
